MCB Merienda Mixes- Online raffle promo mechanics

1. Customers who avail of any of the MCB Merienda Mixes combination can join the online raffle promo. The receipt of the merienda mixes can only be used as their entry to join the online raffle.
2. The customer must fill out and submit the online form at www.maxschicken.com/mcbmerienda
3. Customer needs to input the following information on the microsite to join the raffle promo;
   1. Email Address
   2. Full Name of the customer who will join
   3. Full Address of the customer
   4. Contact Number
   5. Birthdate
   6. Max's Branch where he brought the MCB merienda mixes
   7. Max`s Official Receipt Number
   8. Total Amount Purchased based on OR # issued to them
   9. Date and time of Purchase
   10. Quantity of the MCB merienda mixes ordered
4. Customer needs to make use of the official receipt (containing the purchase of the MCB merienda mixes) to register an entry. Each official receipt can be registered online for the raffle only once. A customer will receive an email notification (email containing # of entries saved) for every successful entry entered in the online raffle.
5. 5 lucky winners will have the chance to travel and visit Max`s San Francisco USA and watch the Madonna concert live.
6. Customers may submit their online raffle entries until 11:59PM on the last day of the promo. Any receipt containing MCB merienda mixes generated during promo dates can be entered in any of day until the end day of the promo period.
7. A customer may win only once if he enters multiple raffle entries.
8. The raffle draw will be done electronically through a RANDOM.ORG program. The raffle draw will be held on the next day after the end of the promo (September 13, 2012). The draw will be at Max`s Head Office in Makati at 4:00pm. Draw Date: September 17, 2012 Monday 2:00pm
9. The winners drawn are subject to verification. An entry is automatically DISQUALIFIED when it falls under the following instances:
   
   1. Fake/erroneous receipt number entered
   2. False/erroneous declaration of quantity of MCB merienda mixes purchased under the said official receipt.
   3. Receipt used more than once in different promo entries
   If the winning customer is disqualified under the said provisions above then the next reserve entry will be declared the official winner.
10. The winners will be notified through registered mail. He/she will also receive an email notification declaring him/her as a winner. The winners may redeem their prizes until October 1, 2012 in Max`s head office, failure to claim prize 14 days after notice will result to disqualification. Winners must have a valid passport and a valid US visa. Travel to US is from October 6-8, 2012.
    
    If the winner doesn`t have any US visa and a valid passport, winner may process his visa application and passport one week before his schedule flight to US.
    
    Prizes will include the following
    1. 3 days and 2 night hotel accommodation with meals
    2. Plane ticket from and to - Philippines to US to Philippines.

    3. Pocket money for the winners
    4. 2 tickets for the Madonna concert in US
    5. Tickets are all valid for 2 persons.
    * Prize is transferable as long as with the approval of the original winner. But not convertible to cash.
11. To claim the prize, the winner must present a valid ID, registered notice/ mail from Max`s and the official receipt showing the purchase of MCB Merienda Mixes and a valid US visa and a valid passport.
12. Owners, suppliers and employees of Max`s Group of Companies or its franchisees including their relatives up to the second civil degree of consanguinity or affinity are not qualified to enter this raffle promo.

Control measures for the MCB Merienda Mixes - Online raffle promo

1. DTI representative shall be invited to check the computer system for registration, displaying of confirmation message, and drawing of winners. A copy of the PHP files from the said system shall be sealed in an envelope and signed by the DTI representative for safekeeping of Max`s Restaurant, for verification of DTI at any time.
2. DTI representative shall also test and verify the confirmation message displayed on the website after registration. The DTI representative shall check the database of the promo.
3. A printed copy of the form, the online confirmation message, the admin login screen and the report format shall be given to the DTI Representative after verification.
4. The database of entries shall include the following minimum requirements:
   1. Email Address
   2. Full Name
   3. Full Address
   4. Contact Number
   5. Birthdate
   6. Max`s Official Receipt Number
   7. A sequentially-generated unique ID number per entry
5. During the entry period and until the raffle draw, the only way to alter the database will be to submit a raffle entry. No person may alter the database in any other way unless done with explicit permission and oversight from the DTI.
6. Another DTI Representative will be invited to be present during the actual drawing of the raffle winner.
7. Max`s Restaurant will generate a list of raffle entries and transaction records prior to the draw. A copy in disc and/or print format shall be sealed in an envelope and signed by a DTI representative for safekeeping of Max`s Restaurant and for verification of DTI at any time until all prizes are claimed.
8. The winners shall be drawn using a RANDOM.ORG program. DTI and Max`s representatives will draw 5 five winners. Additional 5 winners will be drawn as reserved if any of the 5 winners are disqualified.
9. After the 5 winners are determined, Max`s Restaurant Representative and the DTI Representative using the database of raffle entries and the transaction records on the system will verify them.
10. Should any of the winners be disqualified, the next reserve entry will be the winner and will undergo the same verification.
11. A printed copy of the name of the winners will be given to the DTI Representative after determination.
